Specifications include, but are not limited to: General Services Requirementsa) Register, Screen, and assess clients utilizing the service provider's form.b) Register participants and conduct intake interviews as appropriate usingstandardized intake forms provided by the service providers.c) Assure completeness, accuracy and neatness of completed intake forms.d) Conduct a comprehensive assessment of the client and caregiver situationand needs using an EAD approved assessment tool, including determinationof client's condition (ADL's, IADL's,), and determine the types of services andnumber of units of services needed.e) Establish concurrence among the client, family, and caregivers regardingtheir needs and goals.f) Implement the care plan, which may include:i. Obtaining authorization from EAD. Pre-determined guidelines forauthorization of services may be established with the AAA toprevent backlog of cases.ii. Securing services including, but not limited to, KUPUNA CARE(KC) services.iii. Coordinating services.iv. Arranging for services as necessary.v. Following up on the provision of service, per Triage System Model.(The Triage System Model are a set of criteria for prioritizingseniors' requests).g) Monitor and evaluate the implementation of the care plan per TriageSystem Model.h) Reassess the client and review the care plan on a pre-determinedbasis and or as needed
