Specifications include, but are not limited to: Work under this contract consists of the milling and inlaying of State Route 520 mainline with 12.5 mm. The project limits for this work is in Muscogee County in the area of Mile Post 0.00 to Mile Post 7.8. Contractor shall furnish all other labor, materials, equipment, means of transportation, traffic control and incidentals necessary to perform work. This contract shall be bid per item. All locations are to be milled to a depth of 4 inches and inlayed with 4 inches of RECYCLED ASPH 12.5 MM SUPERPAVE, GP 1 OR 2, INCL BITUM MATL & H LIME. Milling, cleaning, tack, and inlay of asphalt as well as replacing disturbed striping shall be included in the bid price for Patching. All milled areas shall be paved to finish grade before reopening the roadway. The Contractor shall be responsible for the removal and disposal of all Milling and debris. All disturbed traffic striping and markings shall be replaced with like kind and should be included in the bid. Various locations on the route are to be mill and inlayed with 4 inches of 12.5 mm asphalt in one lift. Width of locations may vary from one milling machine width to full lane. The Department will mark the areas with the contractor before work starts. This work shall be performed at night from 8 pm to 5 am. Work times may be altered at the Engineers discretion due to traffic volume. At any time during the milling process if an area is discovered to need more than what is required in this contract, the Contractor shall proceed with the mill and inlay of 4 inches, mark, and notify the Department of the areas. Contractor is responsible for obtaining the exact measurements at each location. On-site inspection of facilities may be scheduled with Brian Johnston, District Contract Manager at 706-646-7521. The Contractor shall notify the Thomaston District Office at 706-646-7521, forty-eight hours prior to beginning work and shall coordinate with other construction or utility companies working in the area. The contractor shall be responsible for locating (or have located) any utilities (such as water, gas, telephone, power) if required. All invoices shall be discussed with the project inspector before being sent to the Department. All invoices sent to the Department shall have the DBE forms attached to the invoice for payment. Miscellaneous Provisions: All traffic control for this operation shall be provided by the Contractor. The contractor shall abide by the Manual on Uniform Traffic Control Devices (MUTCD), the current edition for traffic control guidance, as well as section 150 of the Special Provisions and Standard Traffic Control Detail 9100, 9102, 9106, and 9107 in the Georgia Construction Standards and Details. The Contractor is responsible for setting up all TIRs via Email to TMC-TrafficinteruptionReports@dot.ga.gov and shall report all installation and removal times of any lane closures to TMC in Thomaston at phone # 888-488-4368. All Temporary signs and stands shall be removed from the right of way at the end of each work day.
