Specifications include, but are not limited to: 1.1. Proponent must maintain continuous coverage of a client’s care in accordance with their proposed solution. This includes assigning replacement workers during training, vacations, sick leave, or other absences of the assigned homemaker or personal care aide. A roster of qualified personal care aides must be maintained to allow for such contingencies. Proponent must coordinate his/her aides’ work schedules to ensure that adequate staff are scheduled to provide services as ordered to all clients appropriately certified to receive service. The Proponent is responsible for insuring proper certification of its workers. 1.2. Personal care tasks shall include, but not limited to the following: 1.2.1 Bathing; 1.2.2 Toileting; 1.2.3 Grooming; 1.2.4 Shaving; 1.2.5 Oral hygiene; 1.2.6 Dressing; 1.2.7 Feeding/eating;and 1.2.8 Ambulating and transfer, and medically related activities, including the taking of vital signs only in conjunction with the above tasks. 1.3 Homemaker tasks shall include, but not limited to the following: 1.3.1 Light housekeeping; 1.3.2 Laundry; 1.3.3 Assistance with grocery shopping; 1.3.4 Meal preparation; and 1.3.5 Bill paying. 1.4 Clients will be case-managed and monitored by the Clayton County Aging Program case managers. Case managers shall inform the Proponent of potential clients. 1.5 Some clients, based on their financial situation, will have a “cost share”. Clayton County Aging Program will determine the amount to be contributed by clients in “cost share” cases based on a sliding scale. The Clayton County Aging Program requires that the Proponent collect the cost share amounts from the client. The Proponent is responsible for billing its clients for the cost share, collecting these amounts and subtracting the cost share from the monthly bill sent to the Clayton County Aging Program. A record documenting the collection efforts must be maintained in the client’s file.
