Specifications include, but are not limited to: 1. Must demonstrate experience in providing case management services to older adults and demonstrate fiscal viability in order to successfully deliver case management services to identified Henry County consumers. 2. Case management includes the following tasks: a. Assessment, b. Periodic Re-Assessment, c. Service Plan Development and Coordination, d. Follow up by telephone, email or in person with consumers and caregivers when appropriate and necessary, in accordance with program and service requirements, e. Coordination with other programs and advocacy on behalf of consumers who require assistance in accessing other systems of care, f. Documentation, data collection, data entry and programmatic reporting. 3. Case Management Services should be provided in the following manner: a. Develop a comprehensive Service Agreement and Plan of Care (clients who receive homemaker only), b. Develop a Comprehensive Service Plan, c. Coordinate Service Plan, d. Advocate for Consumer, e. Reassess and Review Service agreement and Plan if Care (if applicable), f. Facilitate Service Termination and Discharge. 4. The specific tasks for the selected provider of General Case Management will include: a. Coordination of Care, b. Change to “Assessment of individual consumer strengths, functional and cognitive impairments, support option preferences and unmet needs, c. Provision of information and referral as well as counseling services to older adults that leads to older adults gaining access to appropriate and available services.
