The purpose of this solicitation is to continue the work of the Men's Health Initiative in partnership with a DeKalb County community partner. The goal is to collaborate on efforts to improve the health outcomes of DeKalb County men by removing barriers to access to preventive health services. Program efforts will raise awareness of male health issues and promote health maintenance through community outreach, education, and screening. Community Partner / Health Clinic shall provide each task: 1. Provide primary care, including wellness health examinations, screening services, education, and chronic disease management for men attending the clinic. 2. Operate the Men's clinic at least one day a week during scheduled hours and provide services to men living in DeKalb County. 3. Facilitate client appointments, including those made for labs, diagnostics, and specialty healthcare providers. 4. Provide medical testing for clients from a prescribed list of tests. 5. Document all patients' plans of care in medical records. 6. Provide monthly quality reports for the Men's Health Clinic.
