Describe Acute Rehab Service experience, including: • Experience for a program within a hospital. • Experience in an academic medical center. • Experience with safety net health systems. 2. Define Implementation and Operational Plan • Overview of proposed relationship with Grady with a delineation of roles and responsibilities. • Key steps for implementation, including a proposed timeline. • Detailed operational plan • Medical staff oversight and alignment. 3. Staffing Development and Plan • Details of the proposed staffing structure, including roles, responsibilities, and necessary qualifications. • Training and development plan for existing and new staff to ensure they are equipped with the knowledge and skills required to provide quality rehabilitation care. • Proposed strategy for staff retention and turnover management. 4. Accreditation • Define preparation for CARF and TJC surveys and accreditation. • Provide examples of successful accreditation in other settings. 5. Quality and Patient Satisfaction Plan • Provide a detailed plan for maintaining and monitoring the quality of patient care in line with relevant regulations and best practices. This should include strategies for continuous improvement and risk management. • Approach to ensuring patient satisfaction. • Process for regular review and update of clinical protocols to ensure they are based on the latest research and guidelines in rehabilitation care. 6. Experience • Please provide case examples (2 each) of working with the following, include approach to working with multidisciplinary teams, volume growth and financial performance. o Hospitals providing equitable care to underserved communities. o Academic medical centers. • Provide 1 – 2 examples of previous experience that distinguishes your company from other who provide management services for acute rehab services. • Please see Appendix B for Cost Proposal guidelines.
