Behavioral Services: Plans intended to modify behavior over time (not including agency’s crisis plans) shall not be developed unless appropriate behavioral assessments are completed, the individual and caregiver are willing to accept this support, and the staff who develop the plans are able to provide follow-up support, replacement activities and training. Appropriate assessment includes a Functional Behavior Assessment (FBA) based on direct observation by the Board-Certified Behavior Analyst (BCBA), interviews with current and/or past caregivers, review of appropriate data and documentation including but not limited to existing Behavior Support Plans, data collected, Health Risk Screening Tool, Supports Intensity Scale, Psychological Assessments, and Clinical Assessment of Behavioral Support Needs. The FBA shall identify the function(s) of all target behaviors along with replacement behaviors that are either functionally equivalent or alternative in nature and based on the individual’s functioning level, preferences, and choices.
