LEGIONELLA The following scope of work sets forth the general requirements for providing Legionella Water Testing for Bay Pines VA Healthcare System. General: Provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide Legionella Testing services for 250 mL Legionella samples bottles each month at the following location: Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 40 samples will be taken each month. Samples shall be taken from water fixtures and ice machines and shall include (10) hot and (10) cold for each building. Sampling will be completed in (6) buildings that are on a rotating schedule such that (2) buildings are sampled each month and every building is sampled quarterly. The exact locations will be chosen by the COR each time. Contractor shall utilize the log sheets provided by the COR to also note temperature, biocide level, sample number, and pH of each sample. A sample log sheet is attached. This log sheet shall be provided to the COR the same day of sampling prior to the contractor leaving the site. The contractor shall conduct sampling evenly throughout the quarter so that it is spread out evenly. General: Provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide Legionella Testing services for 1000 mL Legionella samples each quarter for the purpose of sampling cooling towers during the months of December, March, June, and September: Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 (2) samples total each quarter Lee County Health Care Center, 2489 Diplomat Parkway East, Cape Coral, FL 33909 (2) samples total each quarter The exact locations will be chosen by the COR each time. Contractor shall utilize the log sheets provided by the COR to also note temperature, biocide level, sample number, and pH of each sample. The log sheet shall be provided to the COR the same day of sampling prior to the contractor leaving the site. General: Provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide Legionella Testing services for up to (300) 250 mL Legionella samples bottles on an as needed basis at the following location: Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 The exact locations shall be chosen by the COR each time. Contractor shall utilize the log sheets provided by the COR to also note temperature, biocide level, sample number, and pH of each sample. The log sheet shall be provided to the COR the same day of sampling prior to the contractor leaving the site. Contractor shall respond to the facility within 24 hours of notification from the COR to collect these samples. General: Provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide Legionella Testing services for up to (30) 1000 mL Legionella samples on an as needed basis at the below locations. Contractor must be able to respond to the site within 24 hours of notification from the COR to collect the samples. Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 Lee County Health Care Center, 2489 Diplomat Parkway East, Cape Coral, FL 33909 The exact locations will be chosen by the COR each time. Contractor shall utilize the log sheets provided by the COR to also note temperature, biocide level, sample number, and pH of each sample. The log sheet shall be provided to the COR the same day of sampling prior to the contractor leaving the site. Certifications: Contractor s laboratory shall be certified by the Centers for Disease Control and Prevention (CDC) Environmental Legionella Isolation Techniques Evaluation (ELITE) (COMMENT: Contractor s laboratory ) program, for the entire duration of the contract, as proficient at performing the culture of Legionella from environmental samples. The Contractor s laboratory shall perform Legionella culture testing following culture methods published by the International Organization for Standardization or ISO: International Standard ISO 11731:2017, Water Quality Enumeration of Legionella, Culture methods for the isolation of Legionella and estimation of their numbers in water samples. The Contractor s laboratory in addition to being a member of the CDC-ELITE Program should also be ISO 17025 Accredited by A2LA which is a globally recognized accreditation body that provides comprehensive conformity assessment accreditation services. SPS (STERILE PROCESSING SERVICES) WATER The following scope of work sets forth the general requirements for providing water testing for utility water, critical water (RO water) and steam condensate per the AAMI ST108:2023 standard for Bay Pines VA Healthcare System. The contractor shall utilize the log sheets provided by the COR to also note temperature, biocide level (for utility water only) and sample number of each sample. General: Provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide (1) critical water sample at point of generation (after final filters) and test it per the requirements of table 4 of AAMI ST108:2023 (Conductivity, Total Alkalinity, Total Hardness, pH, Bacteria, Endotoxin and Color/Turbidity) each month of the year at each facility listed below. The sample must be taken on the same day as the previous month, plus or minus (5) days and shall be taken immediately downstream of water treatment processes. (2) times per year (every 6 months) this sample will be tested per the requirements of table 2 of AAMI ST108:2023 (Total Alkalinity, pH, Endotoxin, Total Organic Carbon (TOC), Bacteria, Color/Turbidity, Aluminum, Chloride, Conductivity, Copper, Iron, Manganese, Nitrate, Phosphate, Sulfate, Silicate, Total Hardness and Zinc). (when every 6 month samples (per table 2) are taken the monthly samples (per table 4) shall not be taken) The samples shall be taken at the following locations: Lee County Health Care Center, 2489 Diplomat Parkway East, Cape Coral, FL 33909 Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 General: Provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide (1) utility water sample at point of generation (feed to the RO) and test it per the requirements of table 2 of AAMI ST108:2023 (Total Alkalinity, pH, Endotoxin, Total Organic Carbon (TOC), Bacteria, Color/Turbidity, Aluminum, Chloride, Conductivity, Copper, Iron, Manganese, Nitrate, Phosphate, Sulfate, Silicate, Total Hardness and Zinc) each month of the year at each facility listed below. The sample must be taken on the same day as the previous month, plus or minus (5) days and shall be taken immediately upstream of water treatment processes. Contractor shall respond to the facility within 24 hours of notification from the COR to collect these samples. The samples shall be taken at the following locations: Lee County Health Care Center, 2489 Diplomat Parkway East, Cape Coral, FL 33909 Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 General: Provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide (36) critical water samples for resampling per the requirements of table 4 of AAMI ST108:2023 (Conductivity, Total Alkalinity, Total Hardness, pH, Bacteria, Endotoxin and Color/Turbidity) as needed through the year at each facility listed below. Contractor shall respond to the facility within 24 hours of notification from the COR to collect these samples. The samples shall be taken at the following locations: Lee County Health Care Center, 2489 Diplomat Parkway East, Cape Coral, FL 33909 Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 General: Provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide (6) steam condensate samples for resampling per the requirements of table 4 of AAMI ST108:2023 (Conductivity, Total Alkalinity, Total Hardness and pH) as needed throughout the year at each facility listed below. Contractor shall respond to the facility within 24 hours of notification from the COR to collect these samples. The samples shall be taken at the following locations: Lee County Health Care Center, 2489 Diplomat Parkway East, Cape Coral, FL 33909 Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 CERTIFICATIONS The Contractor s Laboratory shall have all the certifications listed above for Legionella sampling. The Contractor s laboratory shall follow methods obtained from the American Public Health Association for establishing trends in the number of CFU s observed in the routine microbiological monitoring of water for HPC. Testing for Endotoxins shall be performed by means of Limlus amebocyte lysate (LAL) assay. The LAL assay is described in ANSI/AAMI ST72. The Contractor shall perform sampling following the Water Sampling Technique referenced in AAMI ST108. DENTAL UNIT WATER LINE TESTING General: Quarterly, provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide (1) Heterotrophic Plate Count from the dental unit water lines once per month at each facility listed below. The sample must be taken on the same day as the previous month, plus or minus (10) days and shall be taken from each dental unit and each water line within that unit. The samples shall be taken at the following locations: Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 (60) samples total each month Lee County Health Care Center, 2489 Diplomat Parkway East, Cape Coral, FL 33909 (84) samples total each month. Bradenton Community Based Outpatient Clinic, 5520 S.R. 62 Suite 101, Bradenton, FL 34208 (24) samples total each month General: Quarterly, provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide up to 250 Heterotrophic Plate Count tests from the dental unit water lines as requested. The contractor must be able to respond to the site within 24 hours of notification. These will be used for re-sampling following shock treatments of the dental unit or if there is suspicion of an issue in the unit. The samples shall be taken at the following locations: Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 Lee County Health Care Center, 2489 Diplomat Parkway East, Cape Coral, FL 33909 Bradenton Community Based Outpatient Clinic, 5520 S.R. 62 Suite 101, Bradenton, FL 34208 CERTIFICATIONS The Contractor s Laboratory shall have all the certifications listed above for Legionella sampling. The Contractor s laboratory shall follow methods obtained from the American Public Health Association for establishing trends in the number of CFU s observed in the routine microbiological monitoring of water for HPC. COOLING TOWER HPC TESTING General: Monthly, provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide (1) Heterotrophic Plate Count from the cooling towers once per month at each facility and location listed below. The sample must be taken on the same day as the previous month, plus or minus (10) days and shall be taken from each location and sample location on the cooling towers at that location. The samples shall be taken at the following locations: Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 (2) samples total each month Lee County Health Care Center, 2489 Diplomat Parkway East, Cape Coral, FL 33909 (5) samples total each month General: As needed, provide all personnel, labor, equipment, parts, tools, supplies, management, transportation, and other items necessary to provide up to 30 Heterotrophic Plate Count tests from the cooling tower as requested. The contractor must be able to respond to the site within 24 hours of notification. These will be used for resampling following shock treatments of the cooling towers or if there is suspicion of an issue in the unit. The samples shall be taken at the following locations: Bay Pines VA Medical Center, 10000 Bay Pines Blvd, Bay Pines, FL 33744 Lee County Health Care Center, 2489 Diplomat Parkway East, Cape Coral, FL 33909 CERTIFICATIONS The Contractor s Laboratory shall have all the certifications listed above for Legionella sampling. The Contractor s laboratory shall follow methods obtained from the American Public Health Association for establishing trends in the number of CFU s observed in the routine microbiological monitoring of water for HPC. 3.PERFORMANCE/TECHNICAL REQUIREMENTS: SAMPLING REQUIREMENTS The Contractor shall follow all healthcare safety and infection control practices and may be required were a protection gown, adorn scrubs, or wear other PPE as required to enter certain areas of the hospital. For Legionella contractor shall take a first draw sample, not allowing the water to flush substantially prior to drawing the sample, unless otherwise directed by the COR. For SPS and Dental samples outlets shall be treated with a general disinfectant prior to sampling and be completely dried to prevent contamination of the collected sample disinfectant residual from the sample tap then flushed for at least 60 seconds before sample is aseptically collected. SAMPLE PREPARATION AND STORAGE FOR TESTING SAMPLES Contractor shall supply each Government facility with its commercial laboratory reference test manual to ensure that the collection and storage of samples are in accordance with Contractor's requirements. The contractor shall provide all materials necessary to collect and preserve samples that are destined to the commercial laboratory for testing. These materials include those items that are dictated by and in compliance with the collection requirements of the commercial laboratory. Contractor shall be responsible for providing the Government facility with acceptable days for collecting and shipping samples, so that all samples arrive at the lab within the shortest time possible, and no later than 24 hours after sample collection. TRANSPORTATION SERVICES FOR TESTING SAMPLES Contractor shall provide all necessary supplies for samples to be transported from the government's originating facility to the Contractor's laboratory. These supplies shall include, but may not be limited to: Shipping and packaging containers. These containers should be insulated to ensure samples stay within required temperature range during shipping. Required labels and packaging materials for shipping samples via courier. Test request forms, preprinted with the appropriate Shipping Section details and account information. SPECIMEN TESTING GENERAL The contractor and/or subcontractor shall perform the type of testing outlined above. The contractor shall provide the minimum and maximum turnaround times from the date of shipping the water samples to the date test results are received by the facility. The contractor shall ensure the accurate and timely performance of laboratory testing services in accordance with the turnaround times identified by the contractor. LEGIONELLA Analytical results shall be reported as Total Legionella (all species). The following sub-groups should also be reported if detected which combined make up Total Legionella (all species): Legionella pneumophila Serogroup 1 Legionella pneumophila Serogroups 2-15 (combined) Other Legionella species (all non-pneumophila species) SPS UTILITY AND CRITICAL WATER TESTING Results shall follow the units defined in AAMI ST108:2023, table 2 or table 4 for the water sample. (Utility water, Critical water or Steam condensate) DENTAL UNIT WATERLINE TESTING Results shall be reported in CFU/ml of heterotrophic water bacteria. REPORTING TESTING RESULTS The results of testing shall be reported within the prescribed turnaround times provided by the Contractor but shall not exceed 14 business days. Bacteria results for SPS Utility and Critical samples shall be reported within 72 hours (3 business days) of receipt of the samples to the lab for testing. This applies for sections 2a, 2b, 2c, 2d, 2e, 2g, 2h and 2i above. A report on laboratory testing results must be issued through email to the designated COR. However, in unusual circumstances where electronic delivery is not possible, the Contractor shall deliver the reports, without an additional charge, by expedited overnight courier shipping, mailing and/or transportation services by hand within 24 hours of results being complete. Test results shall be provided in an editable database (i.e. excel spreadsheet) as well as a PDF on company letter head. Each test report shall, at minimum, include the following information : Submitting facility name Submitting facility account number Test(s) ordered Date/time of sample collection (when available) Date/time test completed Test result Flagged abnormal results Name and address of testing laboratory Name of sampler The contractor shall provide a copy of the Chain of Custody form with the sample results. PERSONNEL: POINT OF CONTACT (POC): The Contractor shall provide a primary Point of Contact who shall be responsible for the delivery and performance of the work. The name of this person, and any alternate(s), to include duty hours and telephone numbers, shall have full authority to act on behalf of the Contractor for the specific order (call) and shall be identified prior to performance . CONTRACTOR FURNISHED ITEMS AND SERVICES. CERTIFICATES, LICENSES, TRAINING AND PERMITS. The Contractor shall, at no additional cost to the Government, obtain all certificates, licenses and permits required for performance of these services, and for complying with all applicable local, state, and Federal laws, rules and regulations. Contractor shall provide training certificates of staff performing the sampling and testing. EQUIPMENT The contractor shall provide own equipment for reading biocide level, temperature, and pH. The contractor shall provide all sampling, testing, and reporting materials and shall provide, annually, the calibration date of equipment to be utilized on site. PPE needed to enter areas of the locations where samples are to be taken shall be provided by Bay Pines VHA.
