Specifications include, but are not limited to: Essential EAP Services EAP services must be available on a 24 hour a day, 7 day a week basis. Toll free telephone and online access for crisis counseling, risk assessment, provider match up services must be available. Provider general information online for members to obtain EAP information. Session Limits: Up to 6 sessions per user per EAP issue/service category per contract year Up to 3 telephonic life coaching (50 minute) sessions per issue per contract year Eligible EAP Issues Include, but are not limited to: Request for Proposal: Employee Assistance Program (EAP) Services Page 4 of 15 ? Stress & Life Management ? Depression, Anxiety, Grief, Loss, Anger, & Violence ? Relationships, Life Changes, Family Matters, Parenting ? Substance Abuse, Addictions, Smoking Cessation ? Career Support & Resiliency Coaching ? Pre-Retirement Planning (Lifestyle Planning) ? Personal Traumatic Events ? Life Coaching EAP Counseling Services should be available on face-to-face basis, telephonic basis, video counseling basis, online chat basis and self-directed online basis. Network/Panel of Providers Provider EAP telephonic customer service and urgent crisis care response counseling on a 24 hour, 7 day a week basis. Non-urgent counseling requests should be accommodated with appointments within 3 business days. Proposer must provide a broad network/panel of providers of various specialties to provide the requested services. (Examples: certified psychologist, marriage and family counseling, grief counseling, etc.)
