The Santa Rosa County School District is seeking Audiology Services to be provided to our students during the 2019-2020 school year. The schedule would be 2 days per week for 46 weeks plus four (4) additional 6-hour days for immediate needs (760 hours total). Provide two price rates, one with mileage included in the hourly rate and another with mileage as a separate rate.

Additional Bidding Instructions

Insurance is required for General Liability, Auto and Worker’s Compensation. Insurance requirements are attached (Attachment A). Direct all insurance inquiries to Risk Management (850) 983-5007.
Contractor must abide by the Jessica Lunsford Law.

Contractor must abide by the School District’s fingerprinting & ID Badge policies. http://www.santarosa.k12.fl.us/humanresources/Fingerprinting. Picture ID to be worn on school sites at all time.
Prior to the start of any work on District property, the contractor must have a current Certificate of Insurance on file with the District Risk Management Office and employee's must have their ID badge in their possession.
