1. Proposals shall include the items listed below addressing the service category of Services for Persons with Disabilities. Any requested changes to the list of services after award of this Contract shall be presented in writing and may require County Council approval. 2. Risk factors for services for persons with disabilities: The following risk factor(s) for services for persons with disabilities shall be considered by respondent in developing their programs: a. Lack of services to match the specific needs of persons with disabilities to promote their desired level of independence (case management, medical services, job and occupational supports, medical surrogates and legal guardianship). 3. List of services as approved by the Children and Families Advisory Board. Proposals shall demonstrate the ability to address the following services: a. Provide individualized case management and navigation services that includes a focus on removing barriers and creating access to an array of services, including: i. Needed medical services (including assistance navigating health insurance and other benefits) ii. Job and occupational supports iii. Independent living supports.
