The proposal should be broken down in the following ways: 1. Lump sum monthly rate (this rate will include all time and materials required to provide preventative maintenance services). 2. Hourly rate for each type of laborer (this rate will include labor rates, truck charges and trip fees). 3. Costs of supplies for review and possible consideration Essential Documents Responses must include the following items in Proposal: 1.Completed Competitive Solicitation Certification Form (Affidavit) (Attachment A). 2.Completed Requested Deviation or Exception Form (Attachment B) if Respondent is requesting deviations to or exceptions from the requirements in the Competitive Solicitation, including language All correspondence must be made through the Vendor Portal.
