Specifications include, but are not limited to: A. Cremation services shall include the following: 1. Transportation to pick up body from funeral homes, the medical examiner's office, hospitals, nursing homes, assisted living facilities, Hospice, or private homes; 2. Transportation of human remains to contractor crematory; 3. Use of crematory facility and equipment; and, 4. Provision of cremated remains to family in a simple container in accordance with applicable Florida statutory and regulatory requirements, or, for cremated remains that remain unclaimed, the contractor shall dispose of the remains pursuant to the requirements of Florida Statutes, Section 497.607(3), as amended or transferred, including any rules or regulations relevant thereto. B. Other responsibilities: 1. Contractor shall submit invoices for cremation services, which shall include: a. Copy of authorization to cremate form signed by legally authorized person as defined in Florida Statutes, Section 497.005(43), if applicable, and; b. Copy of the death certificate; c. Name and address of crematory; and, d. Approval document from county staff authorizing indigent eligibility. 2. Cremations shall be performed within forty-eight (48) hours of the time specified by the County or other legally authorized person for each such cremation in accordance with the requirements of Florida Statutes, Section 497.607(1), as such may be amended or transferred. If the County or other legally authorized person does not specify a date for cremation in a request for cremation, such date shall be deemed the date that is three (3) business days after the County’s written request for cremation. 3. Contractor shall comply with Florida Statutes, Section 497.607(2), as amended or otherwise transferred, and any applicable federal law and regulations regarding the disposition of unclaimed deceased veterans. 4. Contractor, shall, upon receipt of twenty-four (24) hours notice from the County, make its facilities available and open to the County for inspection by the County’s agents, employees, and/or officers. For the purposes of this paragraph, Contractor’s facilities shall include any contracted, rented, leased, or borrowed facilities or equipment used by the Contractor to retrieve, transport, process, store, or incinerate human remains pursuant to this Agreement. 5. Contractor shall not cremate the remains of a deceased person whose identity in not known as defined in Florida Statutes, Section 406.50(4).
