A. The Contractor shall provide a fully insured group vision plan and associated services for employees, retirees, COBRA participants and eligible dependents with an effective date of January 1, 2026. B. The County’s 3rd party administrator will handle COBRA administration and retiree billing. The plan year is January 1 through December 31. C. Annual enrollments occur in September of each year. D. Minimum participation requirements shall be waived. E. An experienced client service manager, with expert support, shall be provided. F. Eligibility for the County’s vision insurance shall be determined on a bi-weekly basis and premiums will be due and payable on a bi-weekly basis. G. The Contractor agrees to allow the County to self-administer payments through biweekly payroll deductions. The County will be remitting the bi-weekly amounts reflected on County’s deductions registers and will include a copy of the register with remittance. H. The Contractor agrees to accept a full eligibility file, generated on a bi-weekly basis, in a HIPAA compliant ANSI 834 transaction set and in the format established and currently utilized by the County. I. The Contractor shall provide educational brochures, plan documents, claim forms, and other materials that clearly explain to participants how to use the vision benefit to its maximum levels in paper and electronic format. The Contractor shall participate in the annual enrollment process to explain the benefits of the program. The County hosts three (3) half-day enrollment meetings.
