Specifications include, but are not limited to: Contractors must satisfy the following mandatory minimum requirements in order to have their Responses evaluated. By submitting a Response, Contractor warrants and represents that it satisfies these requirements. Failure to meet these requirements may result in the Response not being evaluated and being rejected as non-responsive: • Authorized and licensed under Florida Statutes to perform the professional services sought by this RFP. • No material complaints from Consumer Agencies such as The Better Business Bureau. • The proposed personnel to be assigned must have a minimum of 10 years' experience in the Environmental Health Services field. • Submit a detailed fee schedule by category and personnel proposed for the City of Jacksonville account. • The successful vendor must be able to provide Environmental and Health assistance to theRisk Management Division on an as-needed basis including, but not limited to safety program revisions, OSHA updates and compliance, Emergency action Plan (EAP) revisions, EPA and DEP requirements, ergonomic assessments, safety and health training, industrial hygiene sampling, Job Hazard Analysis (JHA), and other health and safety support services including but not limited to mold, asbestos, and lead identification and abatement recommendations. All training and on-site evaluations will be coordinated through Risk Management prior to performance and will include a review of the content, format, and method of training and/or evaluation.
