THE PURPOSE OF THIS SOLICITATION IS TO ESTABLISH A CONTRACT, FOR THE PROVISION OF ICE VENDING MACHINES AT VARIOUS CITY LOCATIONS.
FAILURE TO COMPLETE, SIGN, AND UPLOAD THE FOLLOWING DOCUMENTS WILL RENDER YOUR BID NON-RESPONSIVE:

CERTIFICATION STATEMENT;
CERTIFICATION SECTION; AND
ATTACHMENT B - ANTI-HUMAN TRAFFICKING AFFIDAVIT (MUST BE NOTARIZED).

ALL UPLOADS SHALL BE IN THE PDF FILE FORMAT. NO OTHER FILE FORMAT WILL BE ACCEPTED BY THE CITY.
ATTACHMENT FILES SHALL BE NO MORE THAN 500MB IN SIZE EACH. SHOULD THERE BE A NEED FOR A LARGER SIZE FILE TO BE UPLOADED, SPLIT IN MULTIPLE FILES.
