THE PURPOSE OF THIS SOLICITATION IS TO ESTABLISH A CONTRACT, FOR PLUMBING MAINTENANCE AND REPAIR SERVICES AT CITY OF MIAMI POLICE DEPARTMENT FACILITIES.
FAILURE TO COMPLETE, SIGN, AND UPLOAD THE FOLLOWING DOCUMENTS WILL RENDER YOUR BID NON-RESPONSIVE:

CERTIFICATION STATEMENT;
CERTIFICATION SECTION;
ANTI-HUMAN TRAFFICKING AFFIDAVIT (MUST BE NOTARIZED); AND
ATTACHMENT B-BID PRICE SHEET.

ALL UPLOADS SHALL BE IN THE PDF FILE FORMAT. NO OTHER FILE FORMAT WILL BE ACCEPTED BY THE CITY.
