The Government of the District of Columbia (the District), Office of Contracting and Procurement (OCP), on behalf of the Department of Health Care Finance (DHCF), is seeking a Contractor to provide individualized technical assistance and training/ coaching that will support Medicaid providers’ efforts to provide whole person care to manage the complex needs of Medicaid beneficiaries. The individualized technical assistance and training/coaching will explicitly seek sustained improvement on three core competencies for practice transformation: (1) delivering patient-centered value-based care, including the use of evidence-based approaches; (2) using data and population health analytics to improve care; and (3) supporting providers work together to address a person's physical, behavioral, and social needs.
