Specifications include, but are not limited to: C.5.1.4.2 Work Monday through Friday, except District holidays, from 9:00 am to 9:00pm EST or EDST. C.5.1.4.3 The Contractor shall ensure the HHAC is adequately staffed with a replacement Certified Nurse Practitioners when the contracted personnel are unavailable to work due to, e.g. vacation, sick out, emergencies, etc. C.5.1.4.4 Assess the current medical, psychiatric, psychological, cognitive, emotional and behavioral status and patterns of the patient. C.5.1.4.5 Recognize adverse signs and symptoms and react quickly in emergency conditions. C.5.1.4.6 Treat patients with common acute conditions, illnesses, or minor trauma within accepted protocols and/or in collaboration with a physician. C.5.1.4.7 Obtain medical historical information from patients, as applicable. C.5.1.4.8 Obtain medical historical information from biological families, as available. C.5.1.4.9 Perform rapid laboratory tests, as indicated by medical standards (EPSDT) examination findings, and the age of the patient, including but not limited to anemia, lead, strep, pregnancy, drug screen, STI, urine dipstick, and glucose monitoring. C.5.1.4.10 Provide recommendations for the treatment and/or relief of symptoms related to acute illness. C.5.1.4.11 Prescribe medications as necessary. C.5.1.4.12 Delegate administrative and clinical tasks as appropriate to the Medical Assistant. C.5.1.4.13 Document accurately in the EHR, all patient encounters, historical data and services provided according to Agency and Medicaid standards. C.5.1.4.14 Communicate with caregivers and social workers, as indicated by medical standards (EPSDT) examination findings, and the age of the child, treatment recommendations and follow-up care requirements. C.5.1.4.15 Request consultation or referral with appropriate physicians, clinics or other health care resources as indicated by the medical findings, during the pre-screening examination. C.5.1.4.16 Promote age-appropriate anticipatory guidance, health maintenance, and positive health behaviors during the medical screening process to both child/youth and accompanied adult through education and counseling. C.5.1.4.17 Coordinate follow-up services, to ensure a continuum of care, for identified conditions requiring close monitoring. C.5.1.4.18 Request medical records from community physicians, heath care facilities and hospitals. C.5.1.4.19 Communicate effectively and collaborate with a diverse group of professionals for the purpose of informing the multidisciplinary child welfare team of plans/actions, for teaching/education to benefit the patient/family/caregiver. C.5.1.4.20 Communicate with child/youth patients and families in a manner that encourages the sharing of information in an effort to provide the best care for the child/youth. C.5.1.4.21 Utilize excellent writing skills for both electronic and written communication. C.5.1.4.22 Ensure compliance and standards of care and practice in accordance with all established policies, procedures, guidelines, including HIPAA used in the medical treatment facility.
