Specifications include, but are not limited to: Project Community Alliance for Behavioral Health and Well being (CABHW) aims to improve long-term outcomes for 12-25-year-old Delawareans that have high risk for psychosis (CHR). In cooperation DSCYF, the successful bidder working as the Project Coordinator will (1) increase referral and enrollment of 12-25 year olds into Coordinated Specialty Care program (CSC) for CHR; (2) increase referral and enrollment into CSC for demographic groups with high risk for psychosis and psychotic symptoms (i.e., African American, Hispanic, urban, LGBTQ); (3) improve symptom management for CHR youth and young adults; (4) prevent CHR youth and young adults from converting to psychosis; (5) improve CHR youth occupational functioning; (6) prevent new and ongoing substance use by CHR youth and young adults; (7) reduce critical incidents (e.g., hospitalizations); (8) reduce duration of untreated psychosis for those CHR cases that do convert, and (9) write and implement a sustainability plan for CABHW by grant’s end.
