Specifications include, but are not limited to: The primary goal of care coordination is to facilitate the appropriate, most efficient way to meet the needs of patients to ensure high-quality, high-value health care. To achieve this goal, patient needs, and preferences must be communicated in a timely fashion and to the right people in order to guide the delivery of safe and effective care. When care is poorly coordinated, patients who see multiple providers can face medication errors, hospital readmissions and avoidable emergency department visits. The Care Coordination Platform (hereafter referred to as “the Platform”) is part of an overarching initiative to consolidate various systems and facilitate the delivery of care across multiple DHSS agencies caring for clients with behavioral health disorders. The Platform will help organizations identify, track and better assist clients by removing the barriers of care around social, economic, and behavioral determinants of health. The Platform will also connect with health and social service providers to address the social determinants of health and deliver more integrated care.
