Specifications include, but are not limited to: The State of Delaware Department of Department of Health and Social Services, Division of Public Health, seeks professional services for the Improving Social Determinants of Health through Strengthened Infrastructure. • This Request for Proposal (RFP) is not intended to be used for placement of Community Health Workers (CHW); DPH will be addressing CHW implementation with a separate RFP funding opportunity • The intent of this funding is to go directly to community efforts. This could include supporting staffing and fiscal sponsorship or support for smaller grassroots organizations directly working at the community level that would increase capacity to accept the award while helping to bolster the operations of organization to do more in/for communities. The total amount awarded will be based on the number of proposals received, the number of bids awarded, and the dollar amounts proposed by each successful bidder. No minimum or maximum dollar amount has been determined. Bidders should submit a budget based on the ability to successfully demonstrate the capacity to perform the scope of services requested. The total allocated amount of funding for this RFP is approximately $9 million in total to be distributed among successful bidders. The total amount awarded to each will vary depending on the number of qualified applications submitted. • This funding opportunity is intended to increase or expand the number of policy, system, or built-environment strategies implemented that have a positive effect on the social determinants of health. Examples include an organizational policy that limits the access to unhealthy foods and beverages for its employees and consumers, a neighborhood that employs community garden staff to provide on-the-job training in food preparation that then creates meals for vulnerable residents, or a worksite that offers case management for social determinant of health needs. • Bidders must demonstrate how the proposed work is different and/or expands the organization’s current activities but does not supplant current funding or duplicate existing efforts. The proposal must propose actionable steps towards implementing a systems intervention, and/or policy to reduce the disproportionate impact of negative social determinants on health in communities of color and/or rural communities as they relate to COVID-19 testing, vaccination, and/or treatment. Ensuring equitable access to prevention, mitigation and treatment services requires deploying focused strategies, resources, and activities that meet the needs of targeted individuals. Such strategies include, investing additional resources in historically underresourced or disinvested communities, building infrastructure for healthy behaviors, implementing programs and policies to bring resources to the community rather than forcing community members to strive beyond their means to reach the resources and supporting other place-based solutions that are led by the communities themselves to ensure community goals and needs are met.
