3.1. Orientation/Assessment of Offender. The process for assessing offenders must be presented. Evaluators have an ethical responsibility to conduct evaluations in a comprehensive and factual manner, regardless of the offender’s status within the criminal justice system. The Provider must state how risk/needs will be assessed. A typed report outlining risk and summarizing the above requested information shall be submitted electronically to the supervisor within 30 days after completion of assessment interview. The report shall include primary client needs that, if addressed, will promote success in treatment and on supervision. The report must also include additional supervision and “no-contact” order recommendations to address risk-needs areas identified during the offender evaluation. The offender must be assessed within 30 business days of attendance at an orientation session. The completed assessment form must include recommendation for treatment; date/time of group offender is assigned to, “no-contact” order recommendation, additional needs area(s) and additional supervision recommendations. 3.2. Treatment Plan. The method of developing and documenting a treatment plan for each offender must be described. This shall include the content of the treatment plan, who will be involved in its development, specification of long-term and short-term goals, and the obligations of the treatment staff and the client. A copy of the typed treatment plan will be submitted electronically to the unit supervisor within 30 days of the individual entering into either group or individual treatment. The Provider must coordinate with existing programs and services when practical and beneficial to the offender.
