The State of Delaware is seeking consulting services to support the DMOST program. Objective: Develop public education and DMOST marketing materials to raise awareness of palliative and hospice care Develop and conduct DMOST trainings for health care practitioners, health care providers, emergency care providers, and patients and their families Conduct outreach activities efforts to raise awareness of the DMOST program Coordinate best practices and research with the DMOST Steering Committee, Delaware Health Information Network (DHIN), the National Physician Orders for Life-Sustaining Treatment (POLST) Collaborative, and other key stakeholders Assist the DMOST Steering Committee in the production of an annual DMOST report Assist with maintenance of the DMOST website Provide technical assistance, as needed, to update and/or modify the DMOST form Provide technical support and Subject Matter Expertise, as requested, by DHSS or the DMOST Steering Committee DHSS is soliciting information and recommendations on what type of vendor could support services for the DMOST program.
