All correspondence must be made through the Vendor Portal. Specifications include, but are not limited to: Goal 1: Embed Community Health Workers (CHW) in community-based organizations that are located in and/or serve clients who reside in census tracts with a high social vulnerability index. 1.1 Integrate CHWs into community-based organizations to support the public health response to COVID-19 among priority populations within communities. 1.2 Increase utilization of community resources for those at highest risk for poor health outcomes among priority populations within communities. 1.3 Facilitate engagement of CHW in the care, support, and follow-up across clinical and community settings of priority populations at highest risk for poor health outcomes, including those resulting from COVID-19. Goal 2: Embed Community Health Workers (CHW) in Federally Qualified Health Centers to work with patients who have been disproportionately impacted by health disparities and have underlying health conditions that increase COVID19 risk or are disproportionately impacted by COVID-19. 2.1 Integrate CHWs into care teams to support the public health response to COVID-19 among priority populations within communities. 2.2 Increase utilization of community resources and clinical services for those at highest risk for poor health outcomes among priority populations within communities. 2.3 Facilitate engagement of CHW in the care, support, and follow-up across clinical and community settings of priority populations at highest risk for poor health outcomes, including those resulting from COVID-19. Goal 3: Ensure equitable access to critical resources available to address and support the social determinant of health needs of individuals in census tracts with a high Social Vulnerability Index. 3.1 Ensure Community Health Workers receive training on how to access resources and funds designed to address the social determinates of health and improve the overall health outcomes among priority populations. 3.2 Utilize Community Health Workers to ensure that at risk community members can access funds allocated to address the social determinants of health.
