• Case management services to individuals involved with DFS who are suspected or confirmed to be using alcohol or drugs but are not already receiving services from a community provider focusing on drug and alcohol use. • Liaison services between DFS staff, referred individuals, and community agencies providing support and treatment for substance use. Service #1: Case Management This service will identify substance use or misuse and work with the individual on a plan for treatment that meets the individual’s needs. This also includes a plan for monitoring progress and engaging individuals that are resistant to treatment or who experience relapse by identifying and removing barriers to treatment. This service is for an individual who is suspected or confirmed to be using substances but are not already receiving services from a substance use community provider. Services may include but are not limited to: o screening the individual for substance use treatment o motivating the individual to receive services o identifying and breaking down barriers to treatment o following up on progress in treatment o assisting the individual in case planning needs around substance use o participating in discharge planning o facilitating a support (not treatment) group for individuals using substances o participating in family team meetings
