• Provide all labor, equipment, and materials to conduct full-service septic tank/grease trap pumping, cleaning and disposal (at a State-authorized facility) for all MPS locations at the minimum service frequencies and per the details listed in “Appendix B- Facility List and Schedule.” Approximate locations of tanks/traps are provided in “Appendix C- Locations” • Provide emergency/on-call services during “standard time” within 60 minutes of initial call for service. Provide emergency/on-call services during “premium time” within 120 minutes of initial call for service. • After each service, provide a statement with approximate amount of scum, sludge, and overall condition of tanks and grease traps. • Provide documentation of services provided to the Madison Health Department after each service, using a format approved by the Madison Health Department. • Any manhole cover that is sealed shall be re-sealed with silicone caulking after cleaning occurs.
