The Contractor shall provide custodial services and Perform tasks as described in this Exhibit A (the “Custodial Services”) to the following two (2) Client Agency Sites (collectively or individually as the context requires, the “Facility”) both located within the Client Agency Site located at 1 Long Wharf Drive, New Haven, Connecticut: A. Department of Mental Health and Addictive Services’ (“DMHAS”) Substance Abuse Treatment Unit (“SATU”) has an exclusive exterior entrance with an estimated 7,600 square feet; and B. Department of Children and Families (“DCF”): Suite #121 with an estimated square footage (“SF”) per floor: a. 1st floor – 5,887 SF, b. 3rd floor – 5,958 SF, c. 4th floor – 19,846 SF, and d. 5th floor – 13,411 SF. The Client Agency shall designate a representative(s) to act on behalf of the Client Agency regarding all matters relating to the Custodial Services (the “Client Agency Designee”). The Client Agency shall identify the Client Agency Designee upon the Effective Date of the Contract. 2. Custodial Services Contractor shall: A. Thoroughly complete the Custodial Services in accordance with the Contract, in a professional manner, using quality equipment and materials that conform to all current United States Department of Labor Occupational Safety and Health Administration (“OSHA”) standards and federal, state, and local laws, regulations, and requirements. B. Oversee that the Custodial Services are completed in accordance with the Contract. C. Ensure the Custodial Services conform to the Section concerning Tasks Procedures, of this Exhibit A to ensure the Facility is uniformly clean and hygienic. D. Provide all labor, materials and equipment necessary for cleaning and maintenance, unless otherwise specified herein. E. Have a supervisor monitor its employees’ arrival and departure or utilize a remote timekeeping system for monitoring employee real-time arrival and departure from the Facility. F. Communicate all personnel changes with the Client Agency Designee.
