Description

This Request for Proposal (RFP) is being issued for the purpose of entering into a services and price agreement with at least two contractors to provide designated treating providers – occupational medicine providers for on-the-job injury/illness treatment and other city referrals. Annually, there are approximately 100 employees injured that require medical care.

All services performed must be in compliance with the State of Colorado Workers’ Compensation Statutes, Rules of Procedure and the Medical Treatment Guidelines. All payments for services will be paid per Colorado Workers’ Compensation Rules of Procedure, Rule 18 Medical Fee Schedule.

Additional Bidding Instructions

The City will only accept proposals submitted in the Rocky Mountain E-Purchasing System (RMEPS). The City will not accept late proposals for any reason, including force majeure events and technical difficulties. It is the offeror’s responsibility to submit the proposal in RMEPS prior to the Proposal Due Date and Time. Offerors who do not successfully submit a proposal by the Proposal Due Date and Time will not be eligible for evaluation.
