Proposals are to be received no later than the date and time set forth in the “Schedule of Events,” via emailed PDF file to David Detwiler; ddetwiler@wemberinc.com. Bids received late
will not be considered. Time and date will be based on received time stamp on the email.
• Use the included proposal forms to provide firm information and personnel rates.
• Use the attached excel workbook to provide bids for the project. Bids will be based on the amount of work, in your professional opinion, needed for the project.

Proposals must include, but are not limited to, the following items:
• Letter of interest, including address, telephone number, and email address, and signed by the principal-in-charge.
• Written statement of qualifications with the following information:
o Provide number of years in business as a Materials Testing firm for construction projects. Identify location and size of office(s).
o Identify the key personnel that will be assigned to this project. Include all relevant experience. Include resumes with work experience for all personnel that have been identified and include a list of specific responsibilities for each.
• References – Submit a minimum of three references with email and phone number.
• Description of five projects of similar scope/complexity completed by proposing firm. Include the names of the key personnel.
• Litigation and termination history.
• Both of the following:
o Completed Materials Testing Proposal Form, Exhibit A (below)
o Materials Testing Estimate Worksheet, Exhibit B.
Insurance certificates naming the Owner as additional insured will be required prior to work commencing, but not required as part of this submittal. See “EXHIBIT C: Sample Consultant Agreement” for insurance requirements.
