STATEMENT OF QUALIFICATIONS SUBMITTALS:
Follow the submission directions and submit only the two (2) files identified below:

1 - ORIGINAL SOQ - Addressing all of the items provided below. This version will be evaluated.
1 - REDACTED SOQ - This version will be used to satisfy public records requests. This redacted file may be the same as the original SOQ if your original SOQ contains nothing that is confidential.

Vendor’s Original SOQ is to be no more than 30 pages; any pages beyond this maximum will be deleted and will not be evaluated (i.e. pages 31 through the end of your SOQ will be deleted with no review of content).

The SOQ is to be organized in the order listed below with clear section headings. The entire submission will be counted into the 30 pages (8.5” x 11”), inclusive of all pages and data shared by the Entity. Use of a minimum font size of 11 points and 1-inch margins is required. All pages should be numbered, and the Entity’s name should appear in the header or footer of each page.

Incomplete responses, responses that do not adhere to the page maximum, or responses that do not address each of the following items in the order as outlined below, may be rejected.
