1) Completed Bid Submittal Form, consisting of two (2) pages including:
• Bid Schedule and Full Quote Detail of proposed equipment.
• All Bid pricing is FOB – City of Fort Collins – Streets Dept. 625 9th St., Fort Collins, CO 80524
• Vendor Acknowledgement. You must completely fill out the contact information portion.
2) An overview of your company and a statement confirming the company is an authorized dealer or manufacturer of the equipment.
Failure to submit any of the above may result in your bid being considered non-responsive.
