Description

The Colorado Department of Corrections (CDOC) would like to partner with a vendor(s) to improve the re-entry outcomes for individuals transitioning to the community from the La Vista and Arrowhead Correctional facilities by providing peer recovery coach support services for individuals currently participating in a Therapeutic Community (TC). This project will provide re-entry resources, education, and support services to clients in person or virtually. Individuals will be provided with information on wellness planning, pro-social activities, one-on-one and group coaching, and resources to local community agencies and behavioral health providers. The primary goal is to improve the health, well-being, and equity of all Coloradans through substance abuse education, prevention, wellness planning, and connections to local community agencies, behavioral health providers and MAT providers.
