The Colorado Department of Corrections (CDOC) would like to partner with multiple vendor(s) to improve re-entry outcomes for individuals transitioning into the community from CDOC facilities by providing peer recovery support services. The primary goal is to improve the health, well-being, and equity of all Coloradans through substance abuse education, prevention, wellness planning, Peer Recovery In-Reach coordination and connections to local community agencies, behavioral health providers and MAT providers. These services will be for individuals currently participating in a therapeutic community (TC) and/or individuals that are currently receiving or are eligible to receive MAT services. Individuals will be identified by the Chief of Behavioral Health and Peer Services and/or their designee, to include but not limited to; individuals that are within ninety (90) days of parole eligibility, are within ninety (90) days of release, individuals tabled for release by the Parole Board for program completion, individuals participating in recommended behavioral programs and/or individuals that have been assigned to specialized unit. This project will provide re-entry resources, psycho-education, and support services to clients in person or virtually. Individuals will be provided with information on wellness planning, pro-social activities, one-on-one and group coaching, and resources to local community agencies and behavioral health providers. This project will also serve individuals currently under CDOC community parole supervision to improve re-entry outcomes with increased community behavioral health treatment engagement and parole compliance. Peer recovery support services will provide individuals on parole with substance use disorder education, early prevention, wellness planning, individual or group coaching, resources to local community agencies, behavioral health providers, MAT providers, recovery residences, support groups, referrals to parole community behavioral health clinicians, pro-social activities, and recover community organizations.
