The Contractor shall travel to the CMF to provide the service of measuring patients for medically prescribed specialized wheelchairs due to any additional medical needs of the patients. The Contractor shall submit all documentation with the measurements related to the precise specifications of fabrication of the specialty wheelchairs to the CMF for ordering within 48 hours of services rendered. After each service is rendered, the measurement report and any other associated documentation will be provided to the Contract Manager, or designee, for inclusion in the patient’s medical record as soon as possible to coincide with CDCR/CCHCS policy, which states 24-48 hours.
