TIER 1: 1) Pre-investigative Requirements – The investigator shall administer, review and upload the following: a. Cadet, CHP, Oral Interview. b. Conditional Offer of Employment. c. Background Investigation Questionnaire (BIQ). Contractor shall ensure applicant includes the who, what, when, where, and why when responding to questions on the BIQ. If the applicant fails to include this information, the BIQ shall be returned to the applicant to update their responses. d. Applicant Drug History Questionnaire. 2) Education Verification – The investigator shall inspect official transcripts for all levels of education indicated, High School or above, and include a copy with each investigation. The investigator shall provide a brief summary of the applicant’s education. The education being used to satisfy the minimum requirements shall consist of one or more of the following: a. Graduation from an accredited high school, or accredited/approved private high school. b. Verification of passing GED or high school equivalency. c. Possession of a two-year, four-year, or advanced degree from accredited college or university. 3) Prior Law Enforcement Applications Inquiry – The investigator shall identify all prior law enforcement applications by the applicant. The investigator shall contact every agency and review the applicant’s file, if available. It shall not be assumed any prior investigations were unbiased, thorough, and complete. The investigator shall document the name of the agency, the candidate’s status in the application process, and any relevant or derogatory information discovered. 4) Military Service Evaluation – The applicant’s military history and compliance with the Military Selective Service Act shall be evaluated and documented. a. If the applicant was required to register for selective service, a copy of Selective Service Verification shall be obtained and uploaded to eSOPH. If an applicant was required to register and failed to do so, the reason for failing to register shall be noted and discussed. b. Obtain proof of military service consisting of a “Member 4” or “Service 2” copy of the DD-214, and upload to eSOPH. c. An investigator shall review the applicant’s Official Military Personnel File. d. A brief narrative shall be included detailing what branch the applicant served in (active duty or reserve), dates of service, type of discharge, judicial and non- judicial disciplinary actions, and security clearance issues.
