Specifications include, but are not limited to: The County of Fresno on behalf of the Department of Behavioral Health is requesting proposals from qualified vendors to provide Full Service Partnership (FSP) mental health services and supports, as well as housing services and supports to Transitional Age Youth (TAY) ages sixteen (16) to twenty-five (25) years of age. The TAY Program will deliver integrated mental health and supportive housing services twenty-four (24) hours per day, seven (7) days per week, to a minimum of one hundred forty-nine (149) youth and young adults with serious emotional disturbance (SED), serious mentally illness (SMI), aging out of the Juvenile Justice System, and are at risk of being hospitalized, homeless, and/or incarcerated, and to individuals who are referred by the Fresno County Behavioral Health Court.
