Specifications include, but are not limited to: The Older Adult Community Services (OACS) program is categorized as a State Prevention Program. The OACS program includes four major Prevention Service components. The four components are: Older Adult Mobile Resources Unit: This component will provide bilingual (English/Spanish) and senior appropriate mental health prevention and early intervention and substance use disorder screenings to older adults who are living in geographically and economically isolated areas. This component also incorporates a strategy of Outreach for Increasing Recognition of Early Signs of Mental Illness by reaching out to individuals experiencing early symptoms of a mental illness, so that they can recognize and respond to their own symptoms. The Older Adult Mobile Resources component provides key Prevention Program activities such as reducing risk factors for mental illness in older adults experiencing prolonged isolation, serious chronic medical conditions, severe trauma, ongoing stress, poverty, a mental illness, and or other risk factors experienced by the Older Adult population. This program works to build protective factors by providing access to mental health, physical health, and substance use disorder screening services for geographically, socially or economically isolated or homebound older adults. Older Adult Wellness Services: These services will be focused on delivering comprehensive activities (at least 1 per month, per region) and basic transportation services (e.g., medical appointments, life needs and activity transport) to older adults. Supportive services are also available to older adults who are caring for disabled adults or have custody of children under the age of 18. This program provides access to activities that increase connections with other older adults and provides education on mental and physical wellness. The Older Adult Wellness Services component provides key Prevention Program activities, such as reducing risk factors for mental illness for older adults experiencing serious chronic medical conditions, prolonged isolation, a mental illness, and or other risk factors experienced by the older adult population. This component builds protective factors by providing access to increased activities and basic transportation services (decreasing isolation), providing education on mental and physical health topics increase knowledge of physical and mental health problems and preventative measures as well as knowledge of available services).
