The purpose of the MAI funds awarded under this Request for Proposals (RFP) is to enhance available HIV-related health and support services for eligible YMSM and African American and Latinx transgender women living with HIV infection in Alameda County. The County is issuing the RFP in order to select the bidder best qualified to deliver tailored services to these priority populations. One or more contractors will be funded to offer services in several Service Categories. These will include five Core Medical Services: Medical Case Management, Outpatient Ambulatory Health Services, Early Intervention Services, Mental Health, and Substance Abuse Services. They also will include three Support Services: Non-Medical Case Management, Emergency Financial Assistance and Psychosocial Support. Agencies who apply for Medical Case Management must also apply for Outpatient Ambulatory Health services; these two services are bundled in this RFP. Agencies who apply for Non-medical Case Management must also apply for Substance Abuse services; these two services are bundled in this RFP. Agencies can apply for one or more of the following service categories: Early Intervention Services, Mental Health, Emergency Financial Assistance, and Psychosocial Support.
