Specifications include, but are not limited to: FULL SERVICE PARTNERSHIP SERVICES (FSP) RUHS-BH proposes two FSP programs to TAY (16 – 25 years old) in the Mid-County (San Jacinto/Hemet, Perris, Lake Elsinore, Murrieta/Temecula and adjoining rural areas) and Desert (Beaumont to Blythe) Regions of Riverside County. These programs will provide engagement, behavioral health and substance abuse services, and on-going supports to consumers who are homeless, or at risk of homelessness, with a history of recurring stays at acute or long-term institutional treatment settings without being successfully served through a lower level of care. FSPs align with the goals of MHSA by successfully engaging and supporting consumers that will break the cycle of homelessness, institutionalization and/or incarceration. Each consumer identified as eligible for FSP services will be linked to develop an individualized service and support plan, which is consumer/family-driven, and operationalizes the five fundamental concepts identified above. PROGRAM GOALS Each individual referred by RUHS-BH for the FSP population must be offered an individualized service and support plan. The service and support plans must operationalize the five fundamental concepts identified by Riverside County’s MHSA Plan, i.e., the plans must: Reflect community collaboration in service delivery; Be culturally competent; Be consumer/family driven; Have a wellness/recovery/resiliency focus; and Provide an integrated service experience for the consumer/family. TARGET POPULATION CRITERIA Consumers aged 16-25 years old, who meet all of the following: Have a severe mental illness (SMI) that meets the medical necessity as identified by the current DSM-5 mental illness criteria. The mental disorder must result in substantial functional impairments or symptoms, and/or the person’s psychiatric history must show that, without treatment, there is an imminent risk of decompensation with substantial impairments or symptoms; Due to the substantial functional impairment and circumstances, is likely to become so disabled as to require public assistance, services, or entitlements; and They are in ONE of the following situations: Involved in the criminal justice system, Homeless or at risk of becoming homeless, or Frequent users of behavioral health crisis services (e.g. hospitals, emergency rooms, urgent care, mobile crisis response, etc.) as the primary resource for behavioral health treatment. Note: Traditionally these programs have targeted unserved, underserved, and diverse populations. GEOGRAPHICAL LOCATION OF SERVICES The following are geographical locations within the County of Riverside where the facility(ies) must be located: Mid-County Region : Office location(s) must be in San Jacinto/Hemet, Perris, Lake Elsinore, Murrieta/Temecula and adjoining rural areas. The Mid-County Region serves areas east and south of March Air Reserve Base, including Perris, Lake Elsinore, Murrieta, Temecula, Hemet, San Jacinto, and all other smaller communities around these cities. The goal is to provide accessibility to as many areas in the Mid-County Region as possible. All areas within the Mid-County Region would be eligible for services through the FSP. Desert/Eastern Region: Office location(s) must be in or near Palm Springs, Cathedral City, Rancho Mirage, Palm Desert, Indian Wells, La Quinta, Indio, or Thousand Palms. The Desert Region serves areas east of Moreno Valley, including all cities and small communities found between the Pass area (i.e. Beaumont and Banning) and the Arizona State line (Blythe). The goal is to provide accessibility to as many areas in the Desert Region as possible. All areas within the Desert Region would be eligible for services through the FSP. BIDDER(s) shall describe the physical location(s) (address) from which services will be provided. All service delivery sites must be Medi-Cal certified. Note that not all services provided need to be Medi-Cal reimbursable, but preference shall be given to BIDDER(s) whose proposals maximize Medi-Cal revenue while meeting the requirements of this RFP. Services may be provided at one or more locations. BIDDER(s) shall describe strategies to serve consumers who live within the geographic region, but do not live close to the proposed sites. Proposals shall describe any plans to co-locate staff with collaborative service partners; out-station staff at satellite facilities; use mobile response; or other innovative efforts to optimize consumer access to services and to provide services whenever possible in the community and in normal settings. BIDDER(s)may propose programs for either or both geographic regions. BIDDER(s) proposing both geographic regions are required to submit two separate budgets and provide a description of the difference in services and/or staffing between the two FSPs (if any) that address the unique needs of the geographic areas described.
