Contractor shall provide a diverse range of direct Family Caregiver Support Program (FCSP) services to eligible caregivers throughout San Diego County. This includes support services for family caregivers, including direct services to support family caregivers who provide care to older adults with Alzheimer’s Disease and Related Dementia (ADRD). Services may include: a) Information Services including public information and community education; b) Assistance to caregivers in gaining access to supportive services; c) Support services including counseling, organization of support groups, case management, and training for caregivers in making decisions and solving problems relating to their caregiving roles; d) Respite to enable caregivers to be temporarily relieved from their caregiving responsibilities; and e) Supplemental services including caregiver assessments. Contractor shall perform outreach services to identify potential caregivers and professionals who provide services for the benefit of family caregivers. Contractor shall expand services to develop programs for under-resourced communities and caregivers with the greatest social and economic needs, as defined in Section 9.
