Specifications include, but are not limited to: Santa Clara Valley Health & Hospital System (“SCVHHS”) is seeking proposals (“Proposals) from firms and/or individuals (“Offeror”) to provide dental, vision, hearing screenings, parenting classes, and insurance referral services to parents and children in Santa Clara County in conjunction with the County’s efforts to ensure children receive access to health care coverage and routine and preventative health care. Offerors may apply for one or more services that are being requested. The Offeror would provide broad geographic access and work with the Public Health Department and SCVHHS Administration to review data regarding need to identify which schools/districts should be prioritized, starting with schools which have 50% or more of students eligible for the Free and Reduced Price Lunch Program, prioritizing schools where 75% or more children are eligible for the Free and Reduced Price Lunch Program. As screenings and follow up are provided, the geographic need may shift over time. An annual review of target areas with SCVHHS Administration and Public Health Department for the following year would allow the community’s need to be re-assessed and addressed. Coordination with the Whole Child Initiative will be important to success. To best utilize resources, the Offeror would provide all three (3) health screenings (dental, vision, and hearing) at a school during a period of one (1) week, if possible. This would allow the case management to be done more effectively. Offeror will offer a recommended curriculum and evaluation tool for parenting/caregiver classes. The goal of the classes would be to communicate the value and strategies for healthy lifestyle, eating, communication/interaction, and measure change. Offerors should offer a plan regarding how to connect data from the screenings into electronic health records (beginning with HealthLink, then other types) in order to develop a complete picture of the child’s health status for the medical provider. The plan would need to protect the privacy and security of such data.
