Specifications include, but are not limited to: The objective of the Probation Continuum of Services to Reentry (Pro-CSR) service delivery model is to provide intensive case management to youth reentering the community following an out-of-home commitment. Pro-CSR will provide a continuum of services and opportunities for reentry youth to develop pro-social competency skills and resiliency through family, school, and community involvement through intensive case management and service linkage. Youth, who are eligible to receive individualized services through Pro-CSR, include moderate- to high-risk youth exiting the Juvenile Rehabilitation Facilities or a long-term Juvenile Hall stay, as well as those transitioning home from out-of-home placement. Pro-CSR is designed to build a continuum of services between in-custody and community-based providers, which begins a minimum 60 days prior to and four months post reentry. The Pro-CSR service provider will participate in and facilitate Multi-Disciplinary Team (MDT)/ Child Family Team (CFT) meetings, and share assessment outcomes, case history, and information to promote collaboration between treatment providers. Additionally, the service provider is expected to use the information gathered to effectively design a Transitional Care Plan (TCP) that incorporates the voice and choice of the family and youth. The program will focus heavily on a family-centered team approach, with coordination and communication between the Probation Department, Court, and community stakeholders. Further, the Pro-CSR service provider will assess each individual youth and coordinate an array of services, which may include, but are not limited to, individual, group and/or family therapy, medication accessibility support, crisis response and intervention, safety planning, vocational support, linkage and supports for employment if appropriate, system navigation and advocacy, and on-going service coordination/case management. The service provider should not focus on independently providing all services but rather become a broker of services and collaborate with other organizations to build the capacity for various forms of service delivery.
