Specifications include, but are not limited to: The proposed DRAFT Scope of Work describes the tasks to be performed by the Contractor. The purpose of this project is to schedule, coordinate and facilitate Child and Family Team (CFT) meetings for child(ren)/youth and families who are either already involved with Child Welfare (ongoing) in Sacramento County or who come to the attention of Child Welfare through a report to the CPS Hotline Reporting System (prevention). Services will be provided to youth/families who are not linked to specialty mental health services in accordance with State of California Department of Social Services (CDSS) mandates. Tasks for this project include but are not limited to scheduling, hosting, and facilitating CFT meetings. The use of Safety Organized Practice (SOP) tools, strategies, and framework to engage members of the CFT network to illicit their voice in the development of a shared vision of safety, permanency, and wellbeing is required. Contractors will be responsible for obtaining, maintaining, and reporting on identified data metrics including but not limited to: client demographics, satisfaction of the service, and any systemic challenges, which could include Department staff concerns;for the purpose of monitoring outcomes and utilization of services, as well as for evaluation purposes. PURPOSE AND GOALS OF A CHILD AND FAMILY TEAM (CFT): To include the family and child/youth voice along with their identified natural (informal/formal) support systems in defining goals, strategies and supports. A. Collaborate to garner access to needed services that are culturally responsive to their needs and monitor the child, youth and family’s progress, making individualized adaptations as necessary so that the family’s goals and team-identified outcomes can be achieved. B. Develop an action plan that is created using the strategies, informal and formal supports, that the family agrees will be beneficial, the agency approves of, and that reduces risk and heightens safety for the child/youth and family. C. Facilitate/Plan for how all participants will take part in, support, and implement the action plan developed by the team.
