UC San Diego Health (UCSDH), acting through its Population Health Department, publicly requests information for qualified Hospice care providers currently operating in San Diego County. The Health System wants to understand vendor capabilities in providing General Inpatient (GIP) hospice care, as well as offsite hospice care for stabilized patients. UC San Diego Health currently uses a single Hospice care provider in San Diego County for General Inpatient (GIP) and offsite hospice care services. GIP hospice care includes, but is not limited to, drafting a plan of care, patient stabilization and symptom management, documentation in Epic EMR, and assessment for transfer. Once symptoms are managed and patients are stable for transfer, patients are transferred to an offsite bed for continued hospice care. Hospice services will be performed at the following locations: • GIP: o Jacobs Medical Center o Hillcrest Medical Center o East Campus Medical Center • Offsite: o Available, secured beds throughout San Diego County
