The County of Fresno on behalf of the Department of Behavioral Health (DBH) is requesting proposals from qualified vendors to provide a Transitional Age Youth (TAY) Continuum of Care program. Full-Service Partnership (FSP) including Outpatient (OP)/Intensive Case Management (ICM) services shall be provided to youth and young adults ages 16-25 who meet specialty mental health criteria, are identified as having a Serious Mental Illness (SMI)/Serious Emotional Disturbance (SED) or are identified as at risk of requiring acute care as set forth in the California Welfare and Institutions Code (WIC), section 5600.3(a). Transitional Age Youth (TAY) Continuum of Care shall work with individuals who experience a range of symptoms impacting their functioning in various domains such as school, home, and within their communities and families. Individuals may be at risk of being hospitalized, homeless, and/or incarcerated, or be referred by the County Behavioral Health Court.
