The County of Sonoma is pleased to invite interested parties to respond to a Request for Proposals (RFP) for Therapeutic Foster Care services. The Department is seeking proposals from licensed Foster Family Agencies (FFAs) to provide, facilitate, and oversee Therapeutic Foster Care (TFC) services. TFC is a short-term, intensive, highly coordinated, trauma- informed, and individualized intervention for children or youth with complex emotional and behavioral needs. TFC is available as an Early and Periodic Screening, Diagnosis and Treatment (EPSDT) benefit to children and youth, under the age of 21, who are Medi-Cal eligible and meet medical necessity criteria. Foster children and youth under 21 years of age placed in foster care by the Sonoma County Human Services Department or Sonoma County Juvenile Probation, who meet medical necessity for Specialty Mental Health Services (SMHS), and are at risk of entering a higher level of care, or are stepping down from a higher level of care have been identified by the Department as the priority population. It is estimated 1 to 6 foster children/youth per year could be eligible for this service.
