The Contractor shall provide the following: A. Testing services to be done not later than one (1) week after receipt of fabric sample(s). B. One (1) written report for each sample to be faxed or emailed and one (1) copy to be mailed to the agency location submitting the sample. C. Written report due to each location submitting no later than two (2) weeks after testing of each sample is completed. i. Report identification number (must include Lot number and Roll number of sample), date completed and signature of Contractor. ii. Summary of the sections and tests performed and the outcome of each. iii. Objective of the test, references, and fabric description. iv. Description of the procedures, test requirements and results by section, as well as a statement whether the sample met the requirements of the section (PASS or FAIL).
