Contractor agrees to provide evaluation and calibration, incidental replacement parts and repairs services of BIOS brand “Dry Cal DC-Lite and the Defender Models 510, 510L, 520M & 530H Primary Flow Calibrators and TSI brand Sound Level Meters, Dosimeters, Calibrators, Octave Band Analyzers, Quest Cal Sound Source and Heat Stress Monitors. 1. Contractor shall inspect Industrial Safety and Hygiene equipment upon receipt to determine that equipment is in adequate condition to perform calibration. 2. Contractor shall calibrate equipment as recommended by the manufacturer, per their specification, and provide a certificate of calibration to DIR for each piece of equipment services. 3. After evaluation, the Contractor shall notify DIR Calico Lab by email with results of the inspection and to inform the DIR Calico Lab of the available options for returning the instruments to a fully operational status. No work shall be performed on the instruments until the DIR Calico Lab has been notified and has given its approval. 4. Contractor shall provide service on instruments including both labor and replacement parts’ providing that approval has been granted by the DIR Calico Lab, in accordance with Item “1” above. 5. Contractor shall return said instruments to the DIR Calico Lab, 1555 Doolittle Drive, Suite #140 San Leandro, CA 94577 or to various shipping addresses authorized by the DIR/ Calico Lab. Shipping charges to and from the Contractor will be incurred by the DIR Calico Lab’s choice of previously contracted shipping companies. 6. Shipping locations may be specified by DIR and Calico lab. Primary shipping address(s) may be updated by DIR with a written notification.
