Specifications include, but are not limited to: The City of Los Angeles Department of Aging (LADOA) is designated by the California Department of Aging as Planning Service Area 25, an Area Agency on Aging (AAA) responsible for providing senior and caregiver services to older adults who are 60 years of age or older. Under Title III-D of the Older Americans’ Act of 1965, as amended in 2006, DPHP services provide health risk assessments; routine health screening; nutrition counseling/education services; evidence-based health promotion; physical fitness; group exercise, music, art therapy, dance movement and programs for multi-generational participation, home injury control services, screening for the prevention of depression and coordination of other mental health services; medication management screening and education; gerontological and social service counseling; and education on preventative health services. a. The intent of the Title III-D Evidenced-Based Program for Disease Prevention (EBPDP) is to provide older adults 60+ with services that will: Improve health and promote healthy life styles; Reduce the impact that physical disorders have on the quality of life of older adults; Promote and engage older adults in exercise and other health related activities; and Address issues of incorrect medication and adverse drug reactions. The Contractor is required to tailor an evidence-based service plan to the needs of the participants’ representative of each of the respective Aging Service Areas.
